Termination of a narrow QRS-complex tachycardia: what is the mechanism?
نویسنده
چکیده
A 27-year-old man with a 10-year history of recurrent episodes of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length was 860 msec, and the atrial-His (AH) and Hisventricular (HV) intervals were 80 and 45 msec, respectively. A narrow QRS-complex tachycardia was induced by atrial overdrive pacing. The cycle length of the tachycardia varied between 280 and 350 msec. A single ventricular depolar-

A Narrow QRS Complex Tachycardia:
An 80-year-old man underwent an electrophysiologic procedure because of recurrent episodes of supraventricular tachycardia. Episodes of nonsustained narrow QRS complex tachycardia occurred spontaneously after catheter placement. Examples of the onset and termination of an episode of tachycardia are shown in Figures 1 and 2, respectively. What is the mechanism of the tachycardia? What is the lev...
متن کاملInteresting response of narrow QRS tachycardia to premature atrial complex. What is the mechanism?
Fig.1 shows a premature atrial complex (PAC) introduced during a narrow QRS tachycardia. What is the response and what is the interpretation? The tachycardia is a regular RBBB tachycardia with cycle length of 440 ms and ventriculo-atrial interval of 30 ms. There is near simultaneous activation of atrium and ventricle during tachycardia. Presence of His deflection prior to the QRS makes ventricu...
متن کاملClinical significance of wide QRS complexes at the termination of paroxysmal supraventricular tachycardias.
BACKGROUND A wide QRS complex is not a rare electrocardiographic phenomenon at the termination of paroxysmal supraventricular tachycardia (PSVT), but no plausible underlying mechanism has yet been proposed. The purpose of the present study was to elucidate the frequency and the underlying mechanism of the wide QRS complexes at the termination of PSVT. METHODS We retrospectively reviewed 305 e...
متن کاملWide QRS complex tachycardia in a patient with Wolff-Parkinson-White syndrome and cardiomyopathy: what is the mechanism?
A 32-year old woman with severe mitral regurgitation. congestive heart failure, and Wolff-ParkinsonWhite syndrome developed frequent, nonsustained episodes of a wide QRS complex tachycardia several days postpartum. Figure 1 shows an ECG with telemetry lead I and modified chest lead (MCL) recorded while the patient complained ot" palpitations. The same pattern of initiation and termination of th...
متن کاملWidening of the QRS complex during atrial pacing: what is the mechanism?
An electrophysiologic procedure was performed in a 50-year-old man with a 20-year history of paroxysmal tachycardia. During sinus rhythm, the atrial-His (AH) interval was 70 msec, the His-ventricular (HV) interval was 45 msec, and the QRS width was 80 msec. During atrial pacing at a cycle length of 290 msec, there was widening of the QRS complex (Fig. 1). The transition from a narrow to wide QR...
